Based on your description, it’s likely that the issue was caused by a combination of over-tightening under high torque, use with very heavy weights, and occasional impacts or knocking during workouts. When collars are fastened too tightly, especially under heavy load, and then dropped or bumped, it can push the material beyond its intended design limits—which may lead to breakage over time.

We want to emphasize that this type of failure is extremely rare. In fact, in over 15 years of offering this product, we’ve never received a report of similar damage until now.
